    Weekend Physical Therapist (PT) - Per Diem Float

We are seeking to hire a Weekend Physical Therapist (PT) - Per Diem Float to provide trusted, compassionate, and expert home care to the community. This is a weekend position, Saturday and Sunday, scheduled for 8-hour shifts.

    Job Description

    • Work with clients one-on-one in their home environment focusing on functional mobility, fall prevention, and home safety.
    • Develop and implement creative and evidence-based individual care plans that will enhance clients' quality of life in their own home.
    • Assess client's rehabilitation needs and potential to develop a physical therapy program which may include therapeutic exercises, gait training, use of adaptive equipment, and management of orthotic and prosthetic devices.
    • Collaborate with appropriate members of the health care team to provide physical therapy as well as other services determined necessary.
    • Regularly participate in interdisciplinary case conferences and document patient information as needed.
    • Instruct clients and family members regarding rehabilitative therapy as part of the plan of care resulting in the client/family assuming increased responsibility for care when appropriate.
    • Provide educational programs as required to prepare other corporation personnel to assist in client's plan of treatment.
    • Delegate appropriate tasks/assignments to PTAs.
    • Maintain a productivity average of 4.5 visits a day including completion of all necessary and accompanying documentation within 24 hours.

    Requirements

    • NH PT License (DPT preferred).
    • Home care experience beneficial, although we will train the right individual.
    • Proof of flu vaccination.
    • Reliable transportation.
    • Valid driver's license.
    • Automobile insurance.
    • Physical exam within the last year.
    • Two-step TB test.
