Member Navigator
Job Description:
- Assists with access to care for CSNP members in collaboration with RN case manager and nurse practitioner.
- Obtains information for members based on the model of care requirements.
- Obtains information to assist with evaluation of patient needs.
- Communicate effectively with individuals across the care continuum.
- Documents member information in the electronic medical record.
- Reviews appointments and communicates pertinent information to RN Case Manager(s) and nurse practitioner(s) for additional interventions.
- Collaborate with IDT for coordination of care.
- Communicate with member and/or family, obtaining and providing information regarding care and needs.
- Strong communication skills and focus on customer needs and services.
- Assists with any patient emergency as needed.
- Recommends and/or conducts specific areas of in-service or continuing education with patients and caregiver as appropriate.
- Assists with compliance of quality measures for CSNP program.
- Assist with HEDIS measures and collection of data.
- Assist with collection and presentation of QA data for the CSNP program.
- Participates in ICT meetings for CSNP members.
- Functions in CNA role if needed for member visits.
- Performs other duties as assigned.
Requirements:
- Graduated from an accredited Certified Nursing Assistant (CNA) program with a current CNA certification in the state of practice preferred.
- 2 years' clinical experience as a Certified Nursing Assistant (CNA) in geriatric, adult, or family practice setting, preferred.
- 2 years' experience working in a community-based program involving geriatric, adult, or family practice setting.
- Pertinent clinical work experience relevant to the population served.
- Current BLS certification.
- Must be able to demonstrate computer skills, including use of e-mail and Microsoft Office Word and Excel.
- Must be able to demonstrate effective written communication skills, including but not limited to writing reports and correspondence.
- Must be able to demonstrate effective verbal communication skills, including but not limited to speaking effectively to members, providers, families, and staff, and listening to members, providers, and families.
- Demonstrated ability to meet deadlines, and use time effectively and efficiently.
- Demonstrated successful experience working in an environment of change. Effectively copes with change and uncertainty; responds and "shifts gears" comfortably.
- Is dedicated to meeting the expectations and requirements of both external and internal customers, acts with customers in mind.
- Demonstrated ability to establish and maintain effective relationships with customers, and gain their trust and respect.

- Knowledge of community resources.
- Previous experience working with behavioral health populations.
- Experience working with low-income and disadvantaged populations.
- Previous work with vulnerable adults or senior population.
- Must meet company driving standards.
- Must have a valid state driver's license and be able to drive a car.
- Must meet company's automobile liability insurance standards and provide proof of coverage.
- Must be able to independently travel to assigned locations.
