    Licensed Practical Nurse (LPN)

As an important member of our interdisciplinary team, the LPN delivers quality nursing care to patients through interpersonal contact and provides care and services to allow patients to attain or maintain the highest practicable physical, mental, and psychosocial well-being in accordance with all applicable laws, regulations, and Life Care standards. Your role will involve monitoring and reporting changes in the patient's condition. On a daily basis, you must be able to assign duties, instruct, and provide clinical supervision to CNAs.

    Job Description

    • Provide direct patient care, including catheterization, tube feeding, and patient assessment.
    • Administer medications and perform basic life support as needed.
    • Maintain accurate patient records using EHR systems while adhering to HIPAA regulations.
    • Collaborate with healthcare team members to develop and implement patient care plans.
    • Assist in triage and support patients with developmental disabilities in various settings.
    • Continuously monitor patient conditions and report any changes to the supervising nurse.

    Requirements

    • Valid Licensed Practical Nurse (LPN) license
    • Proficient in patient care and assessment
    • Knowledge of EHR systems and HIPAA regulations
    • Basic life support certification
    • Skills in catheterization and tube feeding
    • Familiarity with long-term care and assisted living environments
    • Experience with IV infusion and critical care preferred
    • Strong communication and interpersonal skills, especially with diverse populations
