Job Description:
- Assist participants in maintaining an environment consistent with their needs.
- Help participants with mobility exercises to enhance independence.
- Provide care across various settings (center or home) ensuring participant satisfaction.
- Assist with daily activities such as walking, transfers, toileting, feeding, and socialization.
- Support the mission and core values of the LIFE Program.
- Participate in participant assessments and reassessments and contribute to the plan of care.
- Attend required meetings and in-services.
- Provide or assist with personal care (bathing, dressing, oral and denture care, etc.).
- Report changes in participant's condition to RN Case Manager.
- Collaborate with staff in implementing services and activities.
- Assist with dining area setup and meal distribution.
- Participate in Interdisciplinary Team meetings.
- Assist with participant transportation and appointments.
- Maintain cleanliness of facilities and participant homes.
- Monitor participant exits and entrances.
Requirements:
- High school diploma or GED.
- Current Pennsylvania State Nurse Aide Certification.
- At least one year of experience with the frail or elderly.
- Dependable transportation with a valid driver's license.
Training and Development:
- Complete annual training on participant needs, body mechanics, and behavior management.
- Participate in sessions aimed at enhancing care quality and job performance.
