Job Description:

• Collaborate with interdisciplinary teams to deliver high-quality, value-based psychiatric care across a variety of clinical settings.
• Focus on proactive, evidence-based treatment strategies that improve the health and well-being of individuals.
• Emphasize adaptability, innovation, and a patient-centered approach to behavioral health care.

Essential Duties and Responsibilities:

• Conduct comprehensive psychiatric evaluations to establish a baseline, manage psychotropic medications, and deliver personalized, need-based care.
• Develop and adjust treatment plans, including medication management, based on patient progress and input from the interdisciplinary team.
• Partner with case managers and interdisciplinary teams to create and refine care plans.
• Participate in regular team meetings to evaluate new referrals and review active cases.
• Contribute to quality initiatives such as performance improvement programs and medication reviews.
• Travel up to 50% of the time to meet with healthcare partners and provide consistent support across clinical settings.

Requirements:

• Master's or Doctoral degree in Nursing from an accredited program specializing in Psychiatric Mental Health.
• Current and unrestricted PMHNP license in the state of PA.
• Board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC).
• Active and unrestricted DEA (Drug Enforcement Administration) license.
• Proficiency in psychiatric evaluations, psychotropic medication management, and evidence-based treatment planning.
• Strong ability to build relationships and work collaboratively with interdisciplinary teams.
• Awareness of social determinants of health and access to resources to address holistic patient needs.
• Comfortable using electronic health records (EHRs), telehealth platforms, and other digital tools.
• Willingness to travel within assigned regions to provide direct care and support to partner organizations.
