    Job Description:

    • Responsible for examination, evaluation, diagnosis, prognosis, intervention, and outcomes of the patient/client as outlined in the New York State Physical Therapy Practice Act.
    • Assists patients to reach their maximum performance and level of functioning, while teaching them to live within the limits of their capabilities.
    • Coordinates, delegates, and supervises responsibilities assigned to supportive staff.
    • Interacts and collaborates with various professionals to provide prevention and wellness services.
    • Participates in operational aspects of the department including performance improvement activities and submitting accurate and timely patient charges and documentation.
    • Engages in hospital infection control measures, equipment training, and organizational programs.

    Core Responsibilities:

    • Obtain a complete patient past medical history and history of present condition.
    • Perform a complete system review of the patient.
    • Complete accurate and appropriate tests and measures.
    • Determine the appropriateness for physical therapy treatment following the examination.
    • Recognize and make appropriate referrals to other practitioners as needed.
    • Determine the physical therapy diagnosis based on the examination.
    • Develop a plan of care specifying interventions, timing, and frequency.
    • Conduct reexaminations to determine changes in patient/client status.
    • Use various physical therapy procedures and techniques aligned with the diagnosis.
    • Observe and evaluate treatment effects, making necessary plan modifications.
    • Perform patient/family education to prevent discharge delays.
    • Treat patients and their families with respect and dignity.
    • Coordinate and direct patient care to ensure policy adherence and needs fulfillment.
    • Determine anticipated goals and expected outcomes for the episode of care.
    • Participate in outcomes data collection and analysis.
    • Maintain accurate and timely documentation.
    • Interact professionally with patients/families, involving them in the care plan.
    • Maintain current patient schedule board or book.
    • Consult other departments to collaborate in patient care and performance improvement.

    Requirements:

    • Graduate of an accredited University or College of physical therapy.
    • License or temporary permit from the State of New York.
