Job Description

• Provide clinical services to patients with functional limitations that affect their ability to carry out activities of daily living.
• Provide direct care, support, and education in the home therapy program under the direction of the attending physician and in collaboration with the family, nursing case manager, home health aide, and any other disciplines.
• Assess referred patients for rehab services.
• Develop and educate patients and their caregivers on the plan of care.
• Thoroughly complete all required documentation.
• Possess excellent interpersonal, communication, and time management skills.
• Willingness to travel throughout our service area.

Requirements

• Licensed as a Physical Therapist by the State of New Jersey.
• Graduate of a Physical Therapist education program.
• Certification for CPR.
• At least 2 years of recent experience.
