Triage Registered Nurse

Mountain Community Health Partnership is seeking a full-time Triage Registered Nurse. Primary responsibilities include but are not limited to enhancing patient care by coordinating communication between patients and providers; handling phone calls from patients who are seeking medical information and/or medical attention; ability to schedule appointments as the patient's condition requires; answering patient questions and providing limited health education; facilitating coordination of care by obtaining and tracking test results and consult records and communication with healthcare providers, community organizations, health plans, facilities, and specialists. The Triage RN will provide patient self-management support for acute and chronic health problems by providing education and medical advice based on pre-approved protocols and evidence-based medicine.

Job Description

• Communicate with healthcare providers, community organizations, health plans, facilities, and specialists
• Provide patient self-management support for acute and chronic health problems

Requirements

• Must have a valid NC Registered Nurse License
• Graduate of an accredited school of nursing
• One year of experience in medical/surgical nursing or clinic setting preferred
• BLS certified preferred
