Nurse Practitioner (NP) Comprehensive Stroke - Days
Reporting to the Stroke Program Coordinator, the Stroke Program Nurse Navigator provides expert nursing consultation and practice oversight, functioning as a liaison between medical staff, nursing staff, family members, and support services. Coordinates multidisciplinary care based on evidence-based practice. Responsible for providing stroke education, delivery, and oversight of patient care, including post-discharge follow-up. Works in collaboration with Stroke Program key stakeholders to meet stroke program certification requirements, including review of data and performance improvement projects. Incorporates the patient experience in all aspects of patient care.
Job Description
- Provide expert nursing consultation and practice oversight
- Function as a liaison between medical staff, nursing staff, family members, and support services
- Coordinate multidisciplinary care based on evidence-based practice
- Deliver stroke education and oversee patient care, including post-discharge follow-up
- Collaborate with Stroke Program key stakeholders to meet certification requirements
- Review data and participate in performance improvement projects
- Incorporate the patient experience in all aspects of patient care
Requirements
- CA RN license
- State of CA Nurse Practitioner licensure (furnishing number in CA)
- DEA furnishing certificate
- BLS certification
- ACLS certification
- NIHSS certification (within 3 months of hire/transfer)
- ASLS certification (within 3 months of hire/transfer)
- ENLS certification (within 6 months of hire/transfer)
- CNRN or NVRN certification
- Master's Degree required
- SCRN preferred
The stroke team is willing to consider an NP with limited NP experience (even new) if they have strong ICU, neuro, and ED experience as a nurse.
