    LPN Field Supervisor

    Independence Healthcare, a Modivcare Personal Care Service, is looking for an LPN Field Supervisor responsible for ensuring quality and safe delivery of home health care services such as supervising caregivers/certified home health aides in a patient's home, evaluating the patient's plan of care, and assessing their physical and psychosocial needs.

    Job Description

    • Performs on-site supervisory and teaching visits to evaluate caregivers
    • Provides clinical orientation to caregivers on assigned home care cases
    • Participates in the training of new nurses and in-services for field staff
    • Performs initial nursing assessments
    • Counsels patient/caregiver in meeting nursing and related needs
    • Assigns and coordinates services provided
    • Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient's condition/needs
    • Prepares clinical progress notes ensuring any changes in the patient's condition are reported
    • Monitors clinical records for complete and accurate documentation

    Requirements

    • Licensed Practical Nurse (LPN)
    • CPR Certified
    • Comfortable driving and performing home visits in the community
