Per Diem Nurse - Field Supervisor
No experience required! We are willing to train. Multicultural Home Care, a Modivcare Personal Care Service, is looking for a Per Diem Nurse as a Field Supervisor responsible for ensuring quality and safe delivery of home health care services such as supervising caregivers/certified home health aides in a patient's home, evaluating the patient's plan of care, and assessing their physical and psychosocial needs.
Job Description
- Schedule: Flexible, will do home visits when aides are in service.
- Pay rate per visit:
- HHA Open $60
- PC Intro $50
- HM Open $40
- Quarterly Supervision: $40
- Visiting Client Territory: Brighton and surrounding areas
- Office: 18R Shepard Street, Brighton, MA 02135
You will:
- Perform on-site supervisory and teaching visits to evaluate caregivers.
- Provide care orientation to caregivers on assigned home care cases.
- Participate in the training of new nurses and in-service training for field staff.
- Perform initial nursing assessments.
- Develop a plan of care and make necessary revisions based upon the patient's condition/needs.
- Document notes ensuring any changes in the service or patient's condition are reported.
- Monitor records for complete and accurate documentation.
- Coordinate visits and communicate changes with the team.
Requirements
- Registered Nurse (RN) preferred; will also consider LPN.
- Feel comfortable driving and performing home visits in the community.
