    Requirements:

    • Graduate of a physical therapy curriculum approved by the American Physical Therapy Association
    • Licensed by the State of Michigan
    • One year of clinical experience preferred

    Specific Duties:

    • Provide initial and ongoing assessments to determine the level of functioning including OASIS assessments at appropriate time points
    • Perform initial and interim assessments and evaluations of the client's level of functioning
    • Identify problems and set goals
    • Develop the plan of treatment in consultation with the physician
    • Provide physical therapy in accordance with the plan of treatment
    • Revise the plan of treatment as needed
    • Coordinate interdisciplinary adherence to the plan of care
    • Provide patient/family education as appropriate
