Requirements:
- Graduate of a physical therapy curriculum approved by the American Physical Therapy Association.
- Licensed by the State of Michigan.
- One year of clinical experience preferred.
Specific Duties:
- Provide services including initial and ongoing assessments to determine level of functioning, including OASIS assessments at appropriate time points.
- Perform initial and interim assessment and evaluation of the client's level of functioning.
- Identify problems and set goals.
- Develop the plan of treatment in consultation with the physician.
- Provide physical therapy in accordance with the plan of treatment.
- Revise the plan of treatment as needed.
- Coordinate interdisciplinary care and adhere to the plan of care.
- Provide patient/family education as appropriate.
