Physical Therapist

This position may be eligible for up to a $10,000 sign-on bonus (restrictions may apply). The Physical Therapist provides comprehensive professional physical therapy service in accordance with the mission and objectives of Nascentia Health. Although this position primarily serves Onondaga County, visits to surrounding counties may be required as needed.

Job Description

• Ensure quality and safe delivery of home health services.
• Provide safe physical therapy interventions to clients/families in accordance with agency policies and procedures.
• Provide client assessments using best clinical practice.
• Develop appropriate plan of care.
• Maintain client care records using approved documentation and format.
• Ensure documentation reflects care and interactions, is completed in a timely manner, and is clear and concise.
• Complete OASIS documentation within regulatory timeframes.
• Follow professional code of ethics and standards of practice.
• Manage a diverse client caseload.
• Communicate client care issues to the interdisciplinary team.
• Assume responsibility for supervision of paraprofessionals relevant to the patient care plan.
• Monitor job performance of home health aides and delivery of home health aide services.
• Comply with Nascentia Health policies and procedures.
• Maintain Nascentia Health productivity standards.
• Adhere to agency policies and procedures including Corporate Compliance, Ethical Standards, and Confidentiality (HIPAA).
• Demonstrate knowledge of federal, state, and local government rules and regulations.
• Demonstrate knowledge and use of infection control practices.
• Adhere to uniform policy and quality improvement standards.
• Keep personnel requirements up to date, including health requirements, professional license requirement, and other Nascentia Health requirements.
• Demonstrate skills competency through periodic evaluations.
• Participate in personal growth and development through education in-services and meetings.
• Maintain and update clinical knowledge on an ongoing basis.
• Present a professional image to clients and the community.
• Perform all other job duties as assigned.

Requirements

• Currently registered by New York State and the American Physical Therapy Association and educational standard board as applicable.
• Current New York State Physical Therapy license.
• Current New York State driver's license and automobile insurance.
• One-year experience as a therapist preferred.

Physical Requirements

• Ability to sit or stand at a desk 90% of the day.
• Ability to perform tasks involving physical activity (including but not limited to): pushing/pulling on average 50 lbs with a max of 150 lbs; lifting/carrying on average 25 lbs with a max of 40 lbs; patient transfers; extensive bending; standing; kneeling; walking in various weather conditions; and ability to go up and down stairs.
• Ability to drive in all weather conditions (rain, sleet, snow, wind).
• Visual/hearing ability sufficient to comprehend written or verbal communication.
• Ability to use telephone and communicate over the phone.
• Frequently uses a laptop computer to enter orders and obtain product information.
• Bloodborne Pathogens Exposure Category I - Employee has tasks that routinely involve exposure or contact with blood and body fluids.

• Competitive salary: This position is an S03 exempt position with a min-max rate of $72,415.71 - $83,922.76 annually with a $10,000 sign-on bonus (restrictions may apply).
