Physical Therapist
This Physical Therapist role is a plan developer. You run the show and make the first impression with patients in starting their care. Our initial plan of care is entirely from your design, and you are the voice of what our patient needs with the referring provider. The table is set for the next therapist to deliver on your design and other disciplines to follow through on the plan. Balancing today's best mobile technology and the support of your office team of experts, you've got it all documented and are on to the next patient no matter the season or whether needing your expertise before lunchtime even hits!
Job Description
- Ensure complete and accurate assessment and delivery of skilled PT services.
- Determine appropriateness of patient for home care.
- Develop patient-centered plan of care based on comprehensive assessment.
- Develop a plan of care using an interdisciplinary approach.
- Ensure that all documentation meets quality standards:
- OASIS evaluations are conducted per CMS guidelines.
- Documentation reflects PT care, treatments, and responses.
- Documentation is clear and concise.
- Documentation is completed in a timely manner.
- Understand and follow PT code of ethics and standards of practice.
- Coordinate with internal and external resources to meet patient needs.
- Maintain established productivity expectations.
- Communicate client care issues to interdisciplinary team including physician.
- Identify needs and refer to additional services as indicated to meet patient goals.
- Report findings and plans to designated Case Manager/Care Team and/or Clinical Supervisor.
- Work in conjunction with Supervisor and designated staff to efficiently manage patients using a team approach.
- Supervise home health aides in performing designated tasks related to patient care and per care plan.
- Communicate appropriate information to home health aide department.
- Comply with Nascentia policies and procedures and quality initiatives.
- Maintain confidentiality.
- Demonstrate knowledge and use of infection control practices.
- Demonstrate knowledge of federal, state, and local government rules and regulations.
- Demonstrate knowledge and compliance with agency policies and procedures.
- Adhere to quality improvement standards.
- Keep personnel requirements up-to-date; i.e., health requirements, licensure requirements, and mandatory in-services.
- Demonstrate proficiency with skills competency through periodic evaluations.
- Participate in personal growth and development.
- Participate in educational in-services and meetings.
- Maintain and update clinical knowledge on an ongoing basis.
- Represent self and agency in a professional manner including compliance with agency dress code.
- Other duties as assigned.
Requirements
- Currently registered by NY State and the American Physical Therapy Association and educational standard board as applicable.
- Current NY State Physical Therapy License.
- Current and valid driver's license and auto insurance, and a vehicle which can be used for work (we do have a Fleet Program for field clinicians).
- Self-directed and organized with the ability to work independently in a fast-paced environment.
- Previous home care/case management experience a plus.
- Proficient computer skills (including knowledge of electronic medical records software) with ability to learn new software.
- Exceptional oral, written, and verbal communication skills.
- OASIS Certification, or willingness to obtain within 1 year of hire.
- Minimum one year experience in Home Health Care required.
Physical Requirements
- Ability to sit or stand for long periods of time in a vehicle or in the field.
- Ability to perform tasks involving physical activity (including but not limited to): pushing/pulling on average 50 lbs with a max of 150 lbs; lifting/carrying on average 25 lbs with a max of 40 lbs; patient transfers, extensive bending, standing, kneeling, walking in various weather conditions, and ability to go up and down stairs.
- Ability to drive in all-weather conditions (rain, sleet, snow, wind).
- Visual/hearing ability sufficient to comprehend written or verbal communication.
- Ability to use telephone and communicate over the phone.
- Frequently uses a laptop computer to enter orders and obtain product information.
