Care Continuum Coordinator

Under the direction of the Supervisor of Care Continuum and in collaboration with the bedside Case Management team, this position processes referrals for patient services to meet their discharge planning needs, assures a cost and quality effective discharge, and always promotes patients' rights and maximum benefits from health care services.

Requirements

• One year in the medical field (i.e., hospital, home health, or nursing home)
• Prefer one year of discharge planning or processing referrals
• Insurance coverage exposure is a plus
• Excellent communication, interpersonal, and organizational skills
• Computer skills including Microsoft Office and electronic health records software
• Required: Bachelor's degree in a related field
