Assertive Community Treatment (ACT) Advocate

As a member of the Assertive Community Treatment Team, you will provide intensive case management services and problem-solving interventions directly and indirectly to client members of this specialized services program, coordinating and maintaining community-based treatment within a team process.
Job Description
- Initiate ongoing assessment, treatment/intervention planning, and scheduling for team members.
- Assess members' emotional well-being, clinical symptomology, behavioral changes, and medication effects.
- Provide counsel and education to members regarding mental illness symptoms, substance use symptoms, and medication information.
- Actively participate in the development of an interdisciplinary plan of service as part of the clinical treatment team.
- Participate in team meetings related to both members and the program.
- Represent the program in the community relative to the needs of the chronically mentally ill and their families.
- Educate and support the community regarding the needs of persons with mental illness.
- Assist in developing effective inter-agency relationships.
- Provide direct clinical care to members and their families through individual and family counseling, co-occurring services, problem-solving interventions, psychoeducation, and support for daily living activities.
- Advocate, counsel, and coordinate regarding daily activities for members in independent community living.
- Provide crisis intervention by participating in 24-hour on-call coverage on a rotating basis.
- Engage in professional development activities to enhance knowledge of mental illness, co-occurring diagnoses, treatment needs, and intervention strategies.
- Secure and maintain appropriate licensure/certification according to Michigan regulations and Medicaid requirements.
- Assist consumers in arranging or providing appropriate transportation for service needs.
- Provide periodic psychosocial assessments of consumers according to program guidelines.
Requirements
- Credentialed by NLCMHA to provide services in this professional discipline.
- Bachelor's Degree in Social Work from an accredited university or a Degree in Nursing.
- Must have licensure with the State of Michigan as a Licensed Bachelors Social Worker. Limited Licensed Bachelors Social Worker or licensed RN considered.

Working Conditions/Physical Requirements
This position may require irregular hours and travel by personal vehicle if an agency vehicle is not available. The physical demands include occasionally lifting up to 25 lbs., stretching/reaching, exposure to bloodborne pathogens and infectious disease, and stooping. Frequently required tasks include hand/finger dexterity, talking in person or on the telephone, hearing, and close vision work.
