    LPN (Licensed Practical Nurse)

    Join our team as a Licensed Practical Nurse (LPN) in a full-time position with rotating weekends. This role is essential in providing quality care to our patients in a rehabilitation center setting.

    Job Description

    • Monitor and document physical and mental health needs
    • Use withdrawal screening, interview, and history taking to assess patient safety
    • Create and update medication administration records and administer medication
    • Work with psychiatric providers to assess mental health symptoms and create treatment plans
    • Assist in maintaining high-quality, timely nursing documentation in patient charts
    • Provide patient education focused on the treatment of mental health and substance use disorders
    • Keep medical supplies in stock and up to date
    • Supervise chemical dependency technician staff

    Requirements

    • Experience working with mental health and substance use disorder is preferred
    • Acknowledge understanding of Client Rights, per MN Rule 31, part 4747.1500
    • Must complete DHS background check and be cleared to work in human services programs
    • Acknowledge freedom from substance abuse problems and ability to comply with NSR drug-free workplace policy
    • Ability to work as part of an interdisciplinary team
    • Must be able to work independently, manage time effectively, and be detail-oriented
    • Possess strong verbal and written communication skills
    • Licensed Practical Nurse in MN in good standing
    • First Aid-CPR-AED/BLS certification
