Hospital at Home Tech
32 hours per week / Shift 9 PM - 7 AM, 3 days per week. Job Description
- Provides patient care within their defined scope of practice that includes patient care, monitoring and observation, in-home patient assessments, clinical and social screenings, clinical interventions, patient education, and participates in telehealth visits from the patient's home while connecting with the physician or virtual command center via Telehealth.
- Works in collaboration with Registered Nurses and other members of the care team in providing direct in-home patient care activities including clinical assessment, obtaining vital signs, assisting patients in need of intervention, and participates in telehealth visits from the patient's home while connecting with the physician or virtual command center via Telehealth.
- Coordinates equipment and supplies.
- Documents appropriate assessment, interventions, and other pertinent patient information in a timely manner.
- Identifies, reports, provides care, and documents patient problems in collaboration with the physicians, nurses, and the technical staff.
- Demonstrates the ability to transport patients safely, utilizing transport devices appropriately.
- Assists physicians, nurses, and technical staff in providing appropriate patient education including the use of appropriate educational materials.
- Interacts with patients, families, nursing, and a variety of interdisciplinary personnel and possesses strong interpersonal and communication skills, a willingness for collaboration within a multidisciplinary team, and clinical proficiency.
- Performs other duties as assigned.
Requirements
- High School diploma or equivalent.
- American Heart Association BLS certification.
- Illinois Emergency Medical Technician Basic (EMT-B) or Illinois Emergency Medical Technician Paramedic (EMT-P).
- At point of hire and annually, required to have a valid driver's license and an annual driving abstract.
Preferred
- ACLS certification.
- PALS certification.
- Applicants are encouraged to acquire community paramedic certification within 24 months of employment.
