Nurse Practitioner

Job Description:

• Perform consultations in environments such as nursing homes and assisted living facilities
• Build an autonomous practice while impacting the lives of older adults and their families
• Work with full administrative support and clinical mentorship
• Provide services on a flexible schedule
• Engage in a competitive, incentive-based compensation model

Requirements:

• Round at assigned facilities and manage acute, subacute, and primary care needs
• Collaborate with the OnSite team, nursing home staff, residents, and families
• Follow up with patients to assess the efficacy of interventions
• Provide education on best practices
• Emphasize non-pharmacological treatments whenever possible
• Master of Science in Nursing with relevant certification (FNP, ANP, AGNP, AGACNP, AGPCNP, or ACNP)
• Active License in South Carolina
• DEA registration
• Excellent communication and documentation abilities
• Strong patient assessment skills
• Strong, ambitious, patient-centric work ethic
