Job Description

• 100% Outpatient No Call Required
• Value-Based Care Model with a panel of Medicare Advantage seniors
• Low Daily Patient Volume: 12-18 patients per day
• Strong Care Coordination Support from experienced clinical and administrative staff

Requirements

• Board Certified/Board Eligible in Family or Internal Medicine
• Colorado medical license (or ability to obtain)
• Experience with Medicare Advantage or value-based care preferred
