Job Description

• Assess patients' home environments for safety and environmental barriers
• Provide fall risk assessments
• Address physical underlying impairments and how they affect overall function
• Train on ambulatory aid and adaptive equipment
• Educate patients and caregivers regarding compensatory strategies and/or a home exercise program

Requirements

• Master's degree in Physical Therapy (minimum requirement)
• 1 year of experience working with older adults preferred
• Home care experience is a plus
• Physical Therapy license (required)

Community Overview

We are a private practice (not an agency) solely focusing on rehabilitation for those that can't make it to an outpatient clinic or opt not to receive therapy in a clinic setting. Patients may or may not be home-bound, low or high functioning. We aim to bridge the gap between a home health agency and an outpatient clinic. We provide all treatment in patients' homes, senior residences, assisted living facilities, or office settings.
