Case Manager

Job Description
- Conduct comprehensive patient assessments to develop individualized care plans.
- Coordinate and manage patient services throughout the care continuum, ensuring adherence to managed care protocols.
- Facilitate discharge planning and follow-up care, while maintaining compliance with HIPAA regulations.
- Collaborate with healthcare teams to review utilization and optimize patient outcomes.
- Utilize strong communication skills to educate patients and families about treatment options and care processes.
Requirements
- CM certification of any type is preferred.
- 2 years of case management experience is required.
- RN license is required.
- BLS certification is preferred.
