Job Description

• Conduct comprehensive patient assessments to develop individualized care plans.
• Coordinate and manage patient services throughout the care continuum, ensuring adherence to managed care protocols.
• Facilitate discharge planning and follow-up care, while maintaining compliance with HIPAA regulations.
• Collaborate with healthcare teams to review utilization and optimize patient outcomes.
• Utilize strong communication skills to educate patients and families about treatment options and care processes.

Requirements

• CM certification of any type is preferred.
• 2 years of case management experience is required.
• RN license is required.
• BLS certification is preferred.
