Job Title

Join our dynamic team as a provider during a maternity leave and sabbatical coverage. This position is essential for maintaining continuity of care in our integrated comprehensive practice model.

Job Description

• Duration: October to February
• Schedule: Really needs someone there every week (will consider 4 days per week)
• Shift: Monday to Friday, 8 AM to 5 PM (no weekends, no extended hours)
• Call: None
• Patient volume: 8-10 patients per day
• Scope: Adult outpatient, integrated comprehensive practice model (no crisis prevention)
• EMR: Epic
• Hospital: Community Health Hospital in town helps with any crisis management
• Group/support: Internal Medicine, Family Practice, Pediatrics, Psychiatry, Licensed Clinical Social Workers

Requirements

• Oregon license required at time of application
• Board certified or board eligible
