RN Nights (6PM-6AM) Full Time
Join our team at Palms Behavioral Health in Harlingen, Texas, where the Registered Nurse is responsible for providing nursing care to patients. This role involves supervising care treatment and services provided by LPNs, CNAs, and other staff on the assigned unit and shift. You will develop, implement, and evaluate an individual plan of care for each patient while communicating appropriate information with the interdisciplinary team.
Job Description
- Provide and coordinate care by assessing physical and behavioral health needs of patients, developing and implementing nursing care plans, maintaining medical records, and educating patients and their families about various health conditions while ensuring patient safety at all times.
- Assess patient's status on an ongoing basis and document pertinent information gathered.
- Ensure nursing treatment plans are specific to assessed needs and complete the initial treatment plan upon admission.
- Take physician's orders (verbal or via telephone) accurately and document appropriately; transcribe and review physician's orders.
- Accurately administer medications per ordered note and document the accurate count of controlled substances; adjust medications under provider's supervision as needed and monitor lab results to validate medication compliance.
- Notify physician of significant changes in status such as difficulties with detox, medical emergencies, or changes in medical status regarding response to medications.
- Supervise self-administration of medications using safety initiatives of using two means of patient identification; document and report effects of scheduled and PRN medications.
- Maintain accountability of administration, recording, control, and security of medications, including narcotics.
- Complete admission and discharge processes efficiently and accurately as directed.
- Confer regularly with the treatment team regarding patient care; attend and participate in unit activities such as therapy groups, family meetings, staffing, and unit meetings.
- Provide education/explanation to patients, families, and co-workers regarding medication usage/maintenance, common side effects, patient's rights, and other topics necessary for optimal patient care.
- Monitor patient acuity, census, staffing needs, and make arrangements for coverage as needed, following established guidelines.
- Assure thorough, accurate documentation is completed for all patients on each shift; complete patient assignment sheets daily and carry out assigned tasks.
- Give thorough report to oncoming staff, supervisors, and physician.
- Provide coverage in emergency services as needed.
Requirements
- Graduation from an approved/accredited school of nursing.
- One or more years' nursing experience within the clinical area of service provided by the facility preferred.
- Experience in behavioral health/psychiatric/addiction recovery environment preferred.
- Valid current RN license from the state or from a compact state, where applicable.
- Certified in Psychiatric Nursing preferred.
- CPR and de-escalation and restraint certification required (training available upon hire).
- First aid may be required based on state or facility requirements.
