Job Description
- Empower individuals to achieve their full potential through supportive person-centered activities that promote safe and sustainable independent living.
- Strengthen community engagement and social support for those served.
- Promote educational activities and health literacy.
- Ensure access to healthy food and nutrition.
- Improve economic stability through exploration of employment-related activities.
- Conduct comprehensive assessments and develop personalized plans to guide each person's unique journey towards independence, recovery, and reintegration.
- Facilitate engaging group therapy sessions within the facility and in the local community, cultivating a sense of shared experience and mutual growth among individuals with diverse abilities.
- Provide one-on-one support as needed to help individuals overcome challenges and develop essential life skills.
- Collaborate with people to establish realistic, measurable, and achievable treatment goals.
- Assess needs, interests, and abilities to develop personalized activity plans.
- Formulate comprehensive treatment plans outlining specific interventions, strategies, and resources necessary for achieving goals.
- Regularly review and update treatment goals and plans in response to client progress or changing needs.
- Facilitate dynamic activities or discussions that promote personal growth, self-awareness, and skill development.
- Implement applicable behavioral support plans and utilize positive strategies to support individuals receiving services.
- Collaborate with a multidisciplinary team to ensure comprehensive care and support for each individual served.
- Complete accurate and professional documentation in a timely manner, including assessments, progress notes, census, billing records, and other related documentation.
Requirements
- Bachelor's degree in human services or related field with three years of field experience; or Master's degree in human services or related field with one year of field experience.
- Experience facilitating group therapy sessions, coordinating activities, and developing treatment plans for individuals with diverse abilities, including those navigating physical or intellectual differences or members of the Deaf community.
- Act 33 and Act 34 clearance and FBI background check, if applicable.
- Valid PA Driver's License.
