Salary Range (Annualized) $76,960-$83,200.
Monitors and coordinates the delivery of health care services for individuals with developmental disabilities who are attending People Inc. Day Programs and living in community-based homes. Supervises the adequate completion of delegated nursing tasks to direct care staff and LPNs. Provides/participates in Agency training.
Job Description
- Monitors and coordinates the delivery of preventive and routine health care for individuals with developmental disabilities in both the day program and residential settings.
- Provides Agency training as required.
- Coordinates and provides health services and training on those services for staff.
- Oversees delegated health-related tasks to Direct Care Staff and LPNs.
- Provides medication training and certification to direct care staff per regulations and Agency policy.
- Adheres to and ensures facility compliance with all physicians, NP, PA, etc. orders.
- Based upon the independent judgment of the RN, has the authority to suspend and/or revoke the responsibility of a Direct Care Staff or LPN to perform or complete any delegated task.
- Completes a physical health and development assessment on individuals every 6 months in day programs.
- In coordination with program managers in day programs, ensures safeguards are updated as needed.
- Completes a monthly breast exam on female clients if specifically ordered at day program by PMD.
- Coordinates medical program needs with community services, medical providers, and other related agencies.
- Ensures adequate scheduling and completion of health-related exams, appointments, laboratory/diagnostic testing as ordered or required.
- Monitors medication administration in the facility.
- Completes a thorough Annual Medication review (Non-ICF) or ensures a pharmacist completes a Quarterly Medication Review (ICF).
- For ICFs, completes quarterly nursing reviews for each person receiving services.
- Updates the Plans of Nursing Services annually and ensures review with staff.
- Participates as an active member of the Interdisciplinary Team.
- Maintains compliance with all applicable State/Federal regulations and Agency policy and procedure.
- Maintains communication with LPN (as applicable), Program Manager, Program Administration, Service Coordinator, and other staff as necessary.
- Full-time residential RNs will follow a rotation schedule for emergency staff training on Saturdays and Sundays.
- Complies with all Agency policy and procedures.
- Other duties as assigned.
Requirements
- Current NYS license to practice as a Registered Nurse.
- NYS Drivers License that meets Agency policy.
- 1 year of related experience.
- Ability to meet physical requirements of the position when performing standing, walking, sitting, bending, stooping, squatting, kneeling, climbing, lifting requirement of 35 lbs.
- Ability to move, reposition, and place individuals in pieces of equipment.
- Physical agility and ability to react to emergency situations, including maintaining Strategies for Crisis Intervention and Prevention (SCIP-R) as per Agency guidelines.
