    Job Description:

    • Responsible for the implementation of standards of care for physical therapy services
    • Completing initial assessments and routine visits
    • Communicating all patient-related information back to the office and documenting accordingly

    Requirements:

    • Currently licensed as a Physical Therapist in the State
    • Preferred two (2) years of experience as an occupational therapist; Home Health experience preferred
    • Good verbal and written communication and organization skills
    • Strong computer skills; familiarity with Allscripts/NetSmart preferred

    Essential Job Functions/Responsibilities:

    • Provide physical therapy services to patients according to a written physician's plan of care
      • Assessing and evaluating therapeutic/rehabilitative/functional status and participation in the development of the total plan of care
      • Evaluating home environment
      • Assessing for muscle strength, mobility, gait, ROM-potential for rehabilitation
      • Directing physical therapy treatment
      • Instructing patients and families/caregivers in the use and care of therapeutic appliances
      • Determining priority needs for physical therapy
      • Reporting to physician patient's reaction to treatment or changes in condition
    • Initiating physical therapy program and instructing other personnel and/or family/caregiver members in certain phases of physical therapy
    • May train patient in the use of prosthetic device
    • Supervising physical therapy assistant, home health aides/personal care aides
    • Identifying patient and family/caregiver needs for other home health services and referring as necessary
    • Preparing and submitting clinical and progress summaries based on the attainment of goals
    • Participating in discharge planning for patient
    • Providing physical therapy consultation to families/caregivers when indicated
    • Providing in-service education for nursing personnel as needed
    • Participating in peer consultation process
    • Other duties as assigned

    Education:

    • Master's (Preferred)

    License/Certification:

    • Physical Therapy License (Required for State of VA)
    • CPR Certification (Required)

    Job Type:

    • Full-time, PRN

    • From $38.00 per hour
