PHH+ a Team Select Company is currently hiring Private Duty Licensed Practical Nurses (LPNs) in the following counties: Wayne, Johnston, Wake, Franklin, Harnett, Onslow.

Job Description:

• Review, initiate, and implement care plan in accordance with the client's needs.
• Provide one-on-one direct nursing care in the home with medically complex clients of all ages.
• Medication management & administration.
• Monitor vital signs & medical equipment.
• Perform physician-ordered interventions, such as tracheostomy or ventilator care when applicable.
• Educate clients and families while providing companionship and emotional support.
• Conduct regular assessments, chart any client changes in condition, progress, & care needs & notify the DON accordingly.
• Assist with other total care nursing duties.
• Make independent clinical decisions when necessary.

Shift Options:

• Day, evenings, overnights.
• Weekends when available.
• FT, PT, & PRN available with most clients.
• 6-12-hour shifts.

Requirements:

• Graduate of an accredited Nursing Program.
• Current, unencumbered RN license.
• BLS Certification.
• Pediatric experience (preferred, but not required).
• High acuity, Trach/Vent experience (preferred, but not required).

Compensation:

• Pay: $30-$33 per hour.
