You must bring with you the desire to be a team member focused on providing personalized care, innovative suggestions, and participation on an interdisciplinary care team for our patients and residents.

Job Description

• Consult and coordinate with the interdisciplinary care team, which includes medical providers and other healthcare professionals to assess, plan, implement, and evaluate individualized patient and resident plans of care.
• Participate in completing baseline and periodic assessments of individualized care needs to ensure the most appropriate care needs are met for each patient and resident.
• Participate in resident care, including accurately and safely administering medications that are ordered by our medical providers, and monitoring to ensure efficacy of medications provided.
• Provide nursing services to short-term patients and long-term residents within the scope of practice, organizational policies, and professional standards of care.

Requirements

• Current license from the state
• Current/active CPR certification, preferred
• Effective verbal and written English communication skills
• Strong attention to detail and accuracy, excellent organizational skills with the ability to prioritize, coordinate, and simultaneously maintain multiple projects with a high level of quality and productivity
• Able to stand and/or walk for more than four (4) hours per day
• Able to bend and/or stoop for more than one (1) hour at a time
