    Job Description

    • Conduct a comprehensive assessment & evaluation using subjective and objective measures.
    • Develop treatment plans and programs in accordance with physician orders.
    • Provide quality treatments, actively progressing goals to achieve the highest quality of life.
    • Conduct screens and provide education to residents/clients, families, caregivers, and staff.
    • Coordinate seamless discharges utilizing the full continuum of therapy and fitness services.
    • Provide accurate and timely documentation & billing in electronic medical record.
    • Demonstrate understanding and knowledge of payer sources and regulatory requirements.
    • Complete and document supervisory visits per regulations.
    • Communicate effectively with residents/clients, families, caregivers, physicians, referral sources, and interdisciplinary team.

    Requirements

    • Graduate from an accredited Occupational Therapy program.
    • Current licensure in the state of practice as required.
    • Competence in Occupational therapy evaluation and treatment.
    • Demonstrated compatibility with PHS's mission and operating philosophies.
    • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.

This new senior community is in a beautiful and serene location atop the bluffs of Dubuque, Iowa, overlooking the Mississippi River. This community offers the following care options: Residential Care Facility (RCF), Residential Care Facility (RCF) with Memory Care, Long Term Care/Care Center.
