We are seeking a dedicated and compassionate Physical Therapist to join our team. The ideal candidate will have a strong background in various physical therapy settings, including outpatient clinics, inpatient facilities, and home health care. This role involves assessing patient needs, developing individualized treatment plans, and providing hands-on therapy to help patients regain mobility and improve their quality of life.

Job Description

• Conduct thorough evaluations of patients to determine their physical therapy needs.
• Develop and implement personalized treatment plans based on patient assessments.
• Provide therapeutic exercises and modalities to enhance recovery.
• Monitor patient progress and adjust treatment plans as necessary.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Educate patients and their families about treatment options and home care strategies.
• Participate in discharge planning to ensure a smooth transition for patients returning home or moving to other care settings.
• Maintain accurate documentation of patient treatments and progress.

Requirements

• Strong knowledge of human physiology and anatomy relevant to physical therapy.
• Experience in various settings such as clinic, home community care, inpatient, pediatrics, outpatient, orthopedics, and home health is preferred.
• Proficient in discharge planning processes to facilitate effective patient transitions.
• Excellent communication skills for interacting with patients, families, and healthcare teams.
• Ability to develop rapport with patients while providing empathetic care.
• Strong organizational skills for managing patient caseloads effectively.
