Nurse Practitioner or Physician Assistant

Seeking a compassionate Nurse Practitioner or Physician Assistant to join a busy, well-established Family Medicine primary care practice. This role provides comprehensive care to patients across the lifespan, from pediatrics through geriatrics, in a collaborative and supportive outpatient setting.

Job Description

• Perform comprehensive medical evaluations, diagnose conditions, and develop individualized treatment plans for patients of all ages
• Manage chronic diseases including diabetes, hypertension, and asthma through ongoing care and medication management
• Collaborate closely with physicians, nursing staff, specialists, and other care team members to deliver coordinated, patient-centered care
• Accurately document patient encounters, histories, and outcomes using EPIC EMR
• Educate patients and families on diagnoses, treatment options, preventive care, and self-management strategies
• Stay current with evidence-based guidelines and clinical best practices
• Maintain a patient-focused approach emphasizing communication, empathy, and quality outcomes
• Average patient volume: 18 patients per day

Schedule & Work-Life Balance

• 4-day work week
• Monday, Tuesday, Thursday, Friday
• 8:00 AM - 4:30 PM

Requirements

• Completion of an accredited Nurse Practitioner or Physician Assistant program
• Active and unrestricted Indiana license (NP or PA)
• Valid DEA, RN/APRN license (for NP) or PA license
• Strong clinical judgment with ability to manage both acute and chronic conditions
• Excellent communication and interpersonal skills
• EPIC EMR experience preferred
