Job Description

• Participates in provider on-call rotation as required.
• Minimum of 1 year experience working with the frail or elderly population is required.
• Not accepting visa candidates.
• Provider must be interested in permanent positions (not accepting locums).

Requirements

• Board Certified in Internal Medicine or Family Medicine - REQUIRED
• Board Certified in Geriatrics (Fellowship-trained) or Certificate of Added Qualifications in Geriatric Medicine (to be completed prior to start) - HIGHLY PREFERRED
• Indiana license (or willing to obtain) - REQUIRED
• Clean malpractice history - HIGHLY PREFERRED
• CDS certification (by time of start) - REQUIRED
• DEA verbal confirmation (by time of start) - REQUIRED
• BLS certification (by time of start) - REQUIRED
• Minimum of 1 year experience working with the frail or elderly population - REQUIRED
• Previous supervisory experience - REQUIRED
• Does the provider require visa support - J1 or H1B? - REQUIRED
