Physical Therapist

Non-profit Hospital in Eastern Oregon is seeking a Physical Therapist for their Home Health setting. This PT will be working with a mostly adult and geriatric caseload. It is an independent PT role, so 1 year of Home Health PT experience is required. The hospital has a great rehab department with a cohesive team of PTs, OTs, and SLPs. The ideal candidate should be comfortable with driving a lot, including in the wintertime. A very competitive compensation package with a sign-on bonus and relocation assistance is available!

Job Description

• Performs and documents initial evaluation and ongoing assessments of patient's condition, adapting treatment plans accordingly
• Develops and implements an individualized plan of care to meet required frequency of visits, which includes patient/family involvement
• Communicates patient progress with the patient, family, and interdisciplinary team
• Educates patients and families on the proposed treatment plan and progress, including skilled recommendations on AE/DME and community support where available and when appropriate
• Supervises activities of assistants, students, and support personnel as needed
• Plans and/or assists in patient discharge
• Ability to work in a typical clinical environment, stamina to remain standing or sitting for extended periods of time, and lift/assist patients as needed
