    Primary Care Physician

    Join our team in Brockton, Massachusetts, located 25 miles south of Boston. This permanent position is available due to organizational growth and offers a hybrid work schedule.

    Job Description

    • Seeking a Board Certified or Truly Board Eligible Family Medicine or Internal Medicine physician
    • Patient population focuses on seniors (65+), Medicare-focused
    • Hybrid schedule: 4 days per week onsite (8:00 AM - 5:00 PM) and 1 day per week remote
    • Monday-Friday schedule with a part-time option of a minimum of 4 onsite days per week
    • Light call requirements: 2 evenings per month and 2 weekends per year
    • Patient volume of 15-18 patients per day

    Practice Model

    Comprehensive, team-based primary care with a strong emphasis on holistic and social support:

    • Routine primary care and chronic disease management
    • In-clinic and virtual care options
    • Care coordination with specialists and hospitals
    • Assistance navigating insurance and the healthcare system
    • Focus on social determinants of health (housing, food security, loneliness)

    Care Team Structure

    Each patient is supported by:

    • Primary Care Physician
    • Gather Guide Partner (social support & community resources)
    • Medical Technician Partner (in-home care and monitoring)

    Additional Practice Details

    • Works alongside other physicians and APPs
    • No APP supervision required
    • EMR: Athena
    • Visa sponsorship not available at this time

    Compensation & Benefits

    • Base salary: $250,000 - $290,000 (DOE)
    • Bonus: 10%
