    Physician Assistant - Annual Health Assessment

    Join a mission-driven team delivering holistic, value-based care to Medicare patients in the Medford, OR area. This permanent, full-time role focuses on annual wellness assessments in both outpatient clinic and in-home settings.

    Job Description

    • Conduct annual Medicare wellness visits in clinic or at patient homes (within a 30-mile radius)
    • Manage high-risk and post-acute patients to reduce avoidable hospitalizations
    • Perform HCC coding, screenings, discharge planning, and medication reconciliation
    • Collaborate with an interdisciplinary team (social workers, care managers, MD advisors)
    • Average 10-12 clinic or 5-6 home visits per day
    • Complete EMR documentation within 72 hours

    Requirements

    • Active Oregon PA license
    • Certified by ANCC or AANP
    • 2+ years NP experience in Primary Care, Family Medicine, Internal Medicine, Geriatrics, or Palliative Care
    • DEA license, CPR/BLS certification, and valid Oregon driver's license
    • Experience with HCC coding, managed care, or home health is a plus
