    Primary Care NP/PA

    Join our dynamic team as a Primary Care NP/PA in the Greater San Francisco Bay Area. Located in Greenbrae, California, this full-time position offers 8-hour day shifts (40 hours per week) within a fast-growing, independent, and integrated healthcare system.

    Job Description

    • Under the supervision and in collaboration with the physician team, deliver high-quality healthcare through comprehensive patient assessment, diagnosis, and treatment.
    • Emphasize health promotion, chronic disease management, and preventive care.
    • Perform in-office procedures, manage patient care plans, and serve as a trusted partner in optimizing patient outcomes within an ambulatory clinic setting.

    Key Responsibilities

    • Conduct thorough patient histories and perform physical examinations.
    • Order and interpret diagnostic and laboratory tests.
    • Identify and evaluate risk factors, chronic conditions, and social determinants of health.
    • Formulate accurate diagnoses and develop individualized care plans.
    • Prescribe or dispense medications in accordance with state and federal guidelines.
    • Educate patients and families on treatment options, medication usage, and preventive health strategies.
    • Provide follow-up care, telephone triage, and ongoing monitoring of patient progress.
    • Collaborate closely with physicians and interdisciplinary teams for continuity of care.
    • Utilize Epic EHR for comprehensive patient documentation.

    Requirements

    • Graduation from an accredited Physician Assistant or Nurse Practitioner program.
    • Current and unrestricted license to practice as a Physician Assistant in the State of California.
    • Board Certification.
    • Active DEA license transferable to California.
    • BLS and ACLS certifications (American Heart Association).
    • Eligible for Medicare and Medicaid enrollment.
    • 2-5 years of clinical practice preferred.
    • At least 2 years of direct experience in the relevant specialty strongly desired.
    • Previous experience in a medical office or outpatient clinic setting preferred.
