Primary Care NP/PA
Join our team in Greenbrae, California, as a Primary Care Nurse Practitioner or Physician Assistant. This full-time position offers 8-hour day shifts in the beautiful Greater San Francisco Bay Area.
Job Description
- Under the supervision and in collaboration with the physician team, deliver high-quality healthcare through comprehensive patient assessment, diagnosis, and treatment.
- Emphasize health promotion, chronic disease management, and preventive care.
- Perform in-office procedures, manage patient care plans, and serve as a trusted partner in optimizing patient outcomes within an ambulatory clinic setting.
Key Responsibilities
- Conduct thorough patient histories and perform physical examinations.
- Order and interpret diagnostic and laboratory tests.
- Identify and evaluate risk factors, chronic conditions, and social determinants of health.
- Formulate accurate diagnoses and develop individualized care plans.
- Prescribe or dispense medications in accordance with state and federal guidelines.
- Educate patients and families on treatment options, medication usage, and preventive health strategies.
- Provide follow-up care, telephone triage, and ongoing monitoring of patient progress.
- Collaborate closely with physicians and interdisciplinary teams for continuity of care.
- Utilize Epic EHR for comprehensive patient documentation.
Requirements
- Graduation from an accredited Physician Assistant or Nurse Practitioner program.
- Current and unrestricted license to practice as a Physician Assistant in the State of California.
- Board Certification.
- Active DEA license transferable to California.
- BLS and ACLS certifications (American Heart Association).
- Eligible for Medicare and Medicaid enrollment.
- 2-5 years of clinical practice preferred.
- At least 2 years of direct experience in the relevant specialty strongly desired.
- Previous experience in a medical office or outpatient clinic setting preferred.
