Post-Acute Care Physician Opportunity

Join our team in Dover, Delaware, for a full-time primary care role that includes patient care rounding in a post-acute care setting at a skilled nursing facility (SNF). This position offers flexibility in your rounding schedule and the opportunity to make a significant impact on patient care.

Job Description

• Round 5 days a week with the flexibility to choose your rounding schedule
• Primary care rounding in skilled nursing facilities
• Manage chronic disease, diagnose illnesses and medical conditions, and implement a plan of care
• Collaborate with clinicians, facility leaders, and others regarding care and services
• Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
• Provide oversight, guidance, and mentorship to our nurse practitioners
• Conduct clinical case reviews as needed and share tools, information, and guidelines as they relate to quality of care
• Minimum of 1-year primary care experience
• Post-acute care experience is a plus
• Active and unrestricted Delaware license or application in process
• Active and unrestricted DEA/prescriptive authority or application in process
• Expected compensation is $210,529 to $270,529 annually with no cap on productivity income potential
• Sign-on bonus is available in months 1 and 13
