Licensed Practical Nurse / Licensed Vocational Nurse

Job Description
- Plan and manage patient care according to each patient's needs.
- Interview patients and record their medical history and physical condition.
- Obtain patient vital signs, including pulse, blood pressure, temperature, and respiration.
- Provide routine care for patients.
- Provide 1:1 care for clients while following the client's plan of care and maintaining their dignity and independence.
- Monitor clients' vital statistics and report abnormal findings to the case manager and physician assessments.
- Perform medication administration, feedings, oxygen, and ostomy care as needed per the client's plan of care and provided assessments of results.
- Complete routines as established by other care professionals such as Physical Therapist, Occupational Therapist, and Speech Therapist.
- Ensure infection control policies are maintained at all times, report possible communicable diseases or infections, and ensure the overall implementation and practice of infection control procedures.
- Accurately document nursing actions of all care given and communicate with family, case manager, and physicians; document the progress and outcomes for established goals and inform physician, case manager, and families of changes in the client's medical condition and needs.
- Order supplies and medications as needed to ensure adequate amounts are on hand at all times.
- Ensure all patient needs are being met in accordance with the instructions of the physician.
- Monitor incidents involving individuals; ensure incident reports are completed timely and accurately, inform case managers, and examine the individual following the incident.
- Ensure all medical equipment is maintained and report faulty equipment to Durable Medical Equipment for repair.
- Perform other duties as assigned.
Requirements
- Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
- Knowledge of individual service plans, including understanding of client rights and health needs.
- Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice.
- CPR Certified.
- Excellent interpersonal skills, including customer service, patience, empathy, and compassion.
- In-depth understanding of patient care policies and regulations.
- Familiarity with symptoms and signs of improvement for several medical conditions.
- Moderate amount of travel between facilities may be required.
