Job Description:

Are you a Physical Therapist looking for a new opportunity? Revival Home Health is seeking a passionate, dedicated Home Health Physical Therapist to join our team in Annandale, VA! How You Will Contribute:

• Provide safe patient care and deliver individualized physician care plans to ensure our patients have an optimal care experience.
• Work as part of an interdisciplinary team, supporting both patients and their families.
• Perform physical therapy evaluation and treatment of patients based upon physician orders/referrals.
• Complete a comprehensive evaluation using appropriate evaluation and assessment tools and procedures.
• Establish measurable goals and develop treatment plans to move the patient toward their maximum level of independence and/or function.
• Regularly re-assess the effectiveness of treatment plans, progress of the patient toward goals, and the need for modifying goals or treatment.
• Communicate patient's needs and progress to the treatment team, physician, person receiving services, and family members, as applicable.
• Consult with medical providers regarding patient evaluation and treatment, progress, and discharge from treatment.
• Recommend and assist in securing special adaptive equipment for patients to improve function.
• Maintain and submit evaluation, treatment, and billing documentation as required by the company, facility, funding source, and regulations.
• Adhere to standards of the company's Code of Business Conduct, policies, and regulations.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities.

Requirements:

• Graduate of a Physical Therapist Education program approved by CAPTE.
• Current Physical Therapist licensure in State and American Heart Association CPR certification required.
• Two (2) years of experience in adult physical therapy; home health experience preferred.
• Excellent observation, verbal and written communication skills, problem-solving skills, and basic math skills.

Professional Development:

• Attend, participate in, and/or conduct internal staff development programs and meetings.
• Obtain continuing education as required by company policy and regulations to maintain and enhance clinical practice skills.

Salary Range: USD $31.00 - $47.00 / Hour
