Home Health Physical Therapist

Location: Annandale, VA

Job Description:

• Provide safe patient care and deliver individualized physician care plans.
• Work as part of an interdisciplinary team, supporting both patients and their families.
• Perform physical therapy evaluation and treatment based on physician orders/referrals.
• Complete a comprehensive evaluation using appropriate evaluation and assessment tools.
• Establish measurable goals and develop treatment plans to maximize patient independence and function.
• Communicate patient's needs and progress to the treatment team, physician, and family members.
• Consult with medical providers regarding patient evaluation and treatment.
• Recommend and assist in securing special adaptive equipment for patients.
• Maintain and submit evaluation, treatment, and billing documentation in required formats and timelines.
• Supervise Physical Therapist Assistants and Aides in patient care activities.

Requirements:

• Graduate of a Physical Therapist Education program approved by CAPTE.
• Current Physical Therapist licensure in State and American Heart Association CPR certification required.
• Two years of experience in adult physical therapy, home health experience preferred.
• Excellent observation, verbal and written communication skills, and problem-solving skills.
