Home Health Physical Therapist

Revival Home Health is seeking a passionate, dedicated Home Health Physical Therapist to join our team in Annandale, VA! Job Description:

• Provide safe patient care and deliver individualized physician care plans.
• Work as part of an interdisciplinary team, supporting both patients and their families.
• Perform physical therapy evaluation and treatment of patients based upon physician orders/referrals.
• Complete a comprehensive evaluation using appropriate evaluation and assessment tools and procedures.
• Review patient records for current diagnosis, past medical history, precautions, and contraindications.
• Establish measurable goals and develop a treatment plan to move the patient toward his or her maximum level of independence and/or function.
• Regularly re-assess the effectiveness of the treatment plan and progress of the patient towards goals.
• Communicate patient's needs and progress to the treatment team, physician, person receiving services, family members, as applicable.
• Consult with medical providers regarding patient evaluation and treatment, progress of the patient, and discharge from treatment.
• Recommend and assist in securing special adaptive equipment for the patient to improve function.
• Maintain and submit evaluation, treatment, and billing documentation in the required format and timelines.
• Adhere to standards of the Company's Code of Business Conduct, policies, Health Insurance Portability Accountability Act, and Corporate Compliance programs.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities.

Requirements:

• Graduate of a Physical Therapist Education program approved by CAPTE.
• Current Physical Therapist licensure in State and American Heart Association CPR certification required.
• Two (2) years of experience in adult physical therapy, home health experience preferred.
• Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills.
