Job Description

• LPNs work under the direction of a registered nurse and are responsible for handling a diverse range of tasks, including administering treatment or medications, performing clinical procedures, documenting clinical data of patients, and more.
• Ensures the care and safety of all residents in the nursing unit.
• Ensures that individual care plans are followed.
• Administers and documents medications, treatments, and other nursing services.
• Makes a count of controlled substances on the assigned nursing unit.
• Records current condition of assigned resident in EMR.
• Receives and gives an accurate report of the resident condition at shift change.
• Communicates to the nursing team and medical provider as indicated and required.
• Assists in the orientation of new staff.
• Maintains competency in all the tasks needed by CMS.

Requirements

• Minimum 18 years of age.
• New York State Licensed Practical Nurse licensure.
• Strong written and verbal communication skills; able to follow written and verbal instructions.
• Basic computer knowledge; PCC e-MAR experience preferred.
• Physical endurance for frequent activity; ability to lift a minimum of 20 pounds.
