Job Description:
We are looking for a committed Physical Therapist - Pediatrics for a Fee for Service position who can cater to patients in Queens to act as the patient's partner throughout the journey of restoring motion and ensuring that the client will function at their personal best. You will maximize the quality of life and movement potential within the spheres of promotion, prevention, diagnosis, treatment or intervention, and rehabilitation.
Responsibilities:
- Provide exceptional physical therapy to patients by assessing and understanding care plans and create physical therapy treatment plans alongside physicians or by prescription.
- Document visits and updates on the patient's care and progress.
- Educate caregivers and their families on how to care for the patient.
- Make the best judgment on the types of care and services needed for each patient.
- Support the company mission by providing the highest quality of home care.
Requirements:
- Proven work experience as a physical therapist.
- Graduate degree or greater in physical therapy.
- Valid license to practice physical therapy.
- Any bilingual skills are a plus!
- Must have at least 1-year experience as a physical therapist in home care - Medicare Part A experience.
- Must have reliable transportation.
- Must have great communication skills, excellent organizational, and time management skills.
- Problem-solving skills.
- Must have great interpersonal skills.
- Excellent computer skills.
- Ability to work independently with minimal supervision.
- Experience using HomeCare HomeBase (HCHB) is a plus but not required.
