Location: Alamosa, CO

Job Description:

• Provide direct rehab care in speech therapy to patients upon physician referral
• Adhere to all applicable laws, regulations, and Life Care standards

Requirements:

• Graduate of an accredited program in speech therapy (minimum MS or MA SLP)
• ASHA certified (CCCs) OR eligible for 9-month Clinical Fellowship Year (CFY)
• Currently registered/licensed in applicable State; maintain active license in good standing
• One year experience in post-acute care or related setting preferred
• CPR certification upon hire or obtain during orientation; must remain current

Specific Job Requirements:

• Participate in community health matters/projects as appropriate
• Proficient in Microsoft Word, Excel, and email
• Liaise with patients, families, and support departments to plan for patient needs
• Demonstrate good body mechanics at all times
• Make independent decisions when necessary
• Knowledgeable of practices, procedures, and laws governing post-acute care facilities
• Implement and interpret department programs, goals, and policies
• Perform proficiently in all competency areas including patient rights and safety
• Maintain professional relationships with associates and vendors
• Maintain confidentiality of all proprietary and confidential information
• Follow company policies including harassment and compliance procedures
• Adhere to Life Care's Code of Conduct

Essential Functions:

• Establish, assess, and modify realistic, measurable, and functional goals
• Oversee and evaluate care given by SLP CFYs (SLP CCC only)
• Follow speech therapy treatment plans under supervision (SLP CFY only)
• Chart appropriately and timely
• Use therapy software accurately
• Exhibit excellent customer service and a positive attitude
• Assist in the evacuation of patients
• Demonstrate dependable, regular attendance
• Use reasoning skills and good judgment
• Communicate and function productively on an interdisciplinary team
• Perform physical tasks such as sitting, standing, bending, and lifting
• Read, write, speak, and understand English
