Location: Alamosa, CO
Job Description:
- Provide direct rehab care in speech therapy to patients upon physician referral
- Adhere to all applicable laws, regulations, and Life Care standards
Requirements:
- Graduate of an accredited program in speech therapy (minimum MS or MA SLP)
- ASHA certified (CCCs) OR eligible for 9-month Clinical Fellowship Year (CFY)
- Currently registered/licensed in applicable State; maintain active license in good standing
- One year experience in post-acute care or related setting preferred
- CPR certification upon hire or obtain during orientation; must remain current
Specific Job Requirements:
- Participate in community health matters/projects as appropriate
- Proficient in Microsoft Word, Excel, and email
- Liaise with patients, families, and support departments to plan for patient needs
- Demonstrate good body mechanics at all times
- Make independent decisions when necessary
- Knowledgeable of practices, procedures, and laws governing post-acute care facilities
- Implement and interpret department programs, goals, and policies
- Perform proficiently in all competency areas including patient rights and safety
- Maintain professional relationships with associates and vendors
- Maintain confidentiality of all proprietary and confidential information
- Follow company policies including harassment and compliance procedures
- Adhere to Life Care's Code of Conduct
Essential Functions:
- Establish, assess, and modify realistic, measurable, and functional goals
- Oversee and evaluate care given by SLP CFYs (SLP CCC only)
- Follow speech therapy treatment plans under supervision (SLP CFY only)
- Chart appropriately and timely
- Use therapy software accurately
- Exhibit excellent customer service and a positive attitude
- Assist in the evacuation of patients
- Demonstrate dependable, regular attendance
- Use reasoning skills and good judgment
- Communicate and function productively on an interdisciplinary team
- Perform physical tasks such as sitting, standing, bending, and lifting
- Read, write, speak, and understand English
