Job Description

• Initiate, develop, and implement the patient Plan of Care (POC). Revise as needed in consultation with the patient, physician, and other team members when appropriate.
• Establish patient-specific goals and incorporate therapeutic, preventative, and rehabilitative nursing services.
• Responsible for the oversight and delivery of quality, patient-centered care rendered in the patient's home or living facility.
• Communicate necessary revisions to patient status and/or needs with the entire care team.
• Regularly consult, advise, and reevaluate patient status with the patient, family, patient representative, and other team members as appropriate regarding safety, plan and outcome of care, and discharge planning.
• Provide complete, accurate, and timely documentation consistent with the plan of care.

Requirements

• Minimum of 1 year of experience as a Registered Nurse with a valid license.
• Current CPR/BLS Certification.
• Valid driver's license and automobile insurance as required by state law.
• Comfortable driving to patient homes for patient visits.
• Prior experience in Home Health, Hospice, Infusion, or Home Care preferred.
