    Licensed Practical Nurse (LPN)

    MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

    Job Description

    • Monitor and support the health and wellness of individuals served
    • Administer medications and maintain accurate documentation
    • Deliver physician-ordered treatments and personal care
    • Conduct assessments and contribute to Individual Service Plans (ISPs)
    • Train staff on infection control, safety, and medication administration
    • Coordinate with physicians and community health professionals
    • Educate individuals on health topics like self-care and smoking cessation

    Requirements

    • Associate's Degree in Nursing or a state-approved LPN certificate
    • Current LPN license in Massachusetts
    • 1 year of nursing experience with similar populations preferred
    • Excellent communication skills and a compassionate approach
