Speech Therapist (SLP)
NeuroRestorative, a part of the Sevita family, provides rehabilitation services for people of all ages with brain, spinal cord, and medically complex injuries, illnesses, and other challenges. Job Description
- Direct and administer speech therapy using techniques that assist in the rehabilitation of individuals with functional disabilities of speech, language, voice, and swallowing.
- Review medical records, test and evaluate clients/consumers, develop treatment plans, and administer therapy.
- Provide effective evaluations, documentation, and communication of medical information.
- Assess speech and language skills, development, articulation ability, and voice and fluency disorders of patients assigned.
- Review medical history, perform evaluation, and determine patient therapy needs.
- Develop, implement, and evaluate individualized patient therapy plans.
- Coordinate therapy modifications to the client's plan of care with the Director of Clinical Services and the client's physician.
- Identify and develop intervention strategies.
- Routinely review pertinent client medical data to determine the effectiveness of therapy services in reaching maximum rehabilitation potential.
- Instruct clients, family members, and primary caregivers as needed to ensure the maintenance of or acquisition of optimal functioning level for each client.
- Ensure effective coordination of client home care services through timely completion of required documentation and transfer of pertinent medical data to the client's physician, Director of Clinical Services, and other caregivers.
- Review and document comprehensive evaluation reports to summarize tests utilized, results of test(s), and diagnosis of speech/hearing disorders.
- Report and record assessments, evaluations, therapy interventions, and patient changes according to physician orders and the Mentor Network's policies.
- Investigate and take appropriate actions on client or employee concerns and/or complaints.
- Maintain appropriate HIPAA compliance and a safe patient environment.
- Perform other related duties and activities as required.
Requirements
- Master's Degree in Speech Therapy or Speech/Language Pathology and one year's experience as a Speech Therapist.
- Current state licensure/certification as a Speech Therapist.
- Travel as needed.
Physical Requirements
- Medium work. Exerting up to 50 pounds of force occasionally, and/or up to 30 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
